Through our Community Resource System Navigation Program, we are able to make a real and positive change in the community by connecting our neighbors to the resources needed to ensure success. Our experienced experts provide a “first response” connection to help our neighbors navigate the changing landscape of health, community and social services.

• Resource Connection: Our team connects individuals with resources needed for self-sufficiency. We partner with neighborhood, housing, food, youth, family and other nonprofit resource organizations to provide information to best help with a query or current need. This is our “Hot-Line” and first line of connection to Project 70Forward. We hope to have this line available 24/7 by 2027.

• Medical/Mental/Behavioral Health Referral: We are connected to two healthcare agencies, Primary Care Center of Carolinas and HealthStar, as well as the Novant Michael Jordan Clinic system for uninsured residents of Mecklenburg County. With these teams of medical professionals, Project 70Forward is able to do direct referrals for all medical, mental and behavioral health services, ensuring that anyone dealing with disabilities can also receive appropriate medical, mental and behavioral health services.
